Masuda City Medical Association Home Care Support Office

This is the first place to turn to when you have concerns about long-term care.
Our care managers will create a care plan to support your life at home and coordinate all necessary services, taking into account the need to reduce the burden on your family."

Eligibility

Individuals covered by long-rerm care insurance who have been certified as requiring support or nursing care and are utilizing home care services

Service Details

1. Services related to the creation of home care service plans
  • Conducting an analysis of the patient's challenges (assessment)
  • Formulation of a home care service plan
2. Benefit management services
  • Continuous monitoring and evaluation of service implementation status
  • Submission of benefit management forms
3. Assistance with administrative procedures
  • Long-term care insurance application procedures (including renewal and certification)

Masuda City Medical Association Home Visit Nursing Station

To help you live with peace of mind in the familiar surroundings of your home, even if you have an illness or disability, our nurses and rehabilitation staff will visit your home to provide medical care, health management, and support for daily living.

Eligibility

This service is available to individuals covered by long-term care insurance who have been certified as requiring support or nursing care and are utilizing home care services.

Service Details

Based on instructions from your primary care physician, a nurse from our station will visit your home to provide nursing and care services, including monitoring your condition, assisting with bathing, shampooing, and sponge bathing (wiping the body for those unable to bathe), meal assistance, toileting assistance, pressure ulcer (bedsore) care, functional training and guidance, and consultation for family caregivers. (standard visit duration is approximately 1-1.5 hours.)
